Horse chestnut seed extract (HCSE) preparations are used for a variety of vein disorders including pain and heaviness of the legs, cramps in the calves, pruritis, leg swelling, varicose veins, edema of the lower limbs and hemorrhoids. A principle constituent is escin (aescin), which is a triterpene glycoside mixture. It has anti-exudative and vascular tightening effects. In double blind, as well as in pharmacological studies, significant improvement has been shown in the symptoms of chronic venous insufficiency. An observation study of 800 general practitioners in Germany, treating over 5000 patients, concluded that leg pain, fatigue, tension, swelling, itching and edema either disappeared, or improved significantly after the administration of horse chestnut seed extract. In a randomized placebo controlled study of 40 patients with venous edema, the effects of HCSE was significant. Measurements of leg circumference showed HCSE to be safe and an effective adjunct to compression therapy. In a clinical study of 22 patients with proven venous insufficiency, capillary filtration was reduced by 22 percent in 3 hours after taking a preparation containing 50 mg of escin. Following German clinical experience, Health Concerns Formula V supplies the recommend 100 mg of escin per day in 3 tablets.
